Schedule an Appointment

Only the patient who makes an appointment can come in and must be wearing a mask. For patients that are minors, one adult is allowed. For cancellations please give us 48 hours notice to avoid a last minute cancellation fee.
Please note that failing to cancel/reschedule your appointment with at least 48 hours notice or not showing up for your appointment will result in a $75 fee being charged to your account.
